Registrar’s Office Forms & Instructions
Transcript Requests
Students who wish to receive or send an official copy of their transcripts, will need to submit a Transcript Request. Transcripts requests must include your name, address, phone number and email address, your SS# or Kirtland ID and Date of Birth, and must include a physical signature. There are two options for requesting a Kirtland Community College transcript. See more information on How to Request a Transcript.
Request to Graduate
Students who wish to graduate from Kirtland need to submit a Request to Graduate form. Upon submission of this form, a degree audit will be done on the student’s academic record to ensure that all requirements have been met. Degree audits will take approximately two weeks to complete. A letter indicating the student’s graduation status will be emailed to their KMail account. Commencement is held the last Friday of the winter semester.
Change of Program
Students need to complete a change of program form to officially change their program of study with the college. Completed forms are submitted to the records/registration office. All students who request to have their major changed will automatically have their advisor changed to the appropriate program advisor.
Go to Student Forms Central to download records/registration forms.
